While there is now a voluminous literature on the role of social, economic, cultural and environmental determinants as they affect health equity, little has been written on the effect of health care bureaucracies and organisational forms on health outcomes. Dr. Hyde asks three major questions:
- What is the role of the health system in contributing to health outcomes and health status?
- Do specific sub-systems of the health system affect health outcomes and health status more positively than others? and
- Does primary health as a specific leading sub-system offer better health outcomes than acute care?
